MAJOR FUNCTION:
The Patient Flow Nurse Coordinator is responsible for coordinating high-acuity patient movement,
supporting system capacity management, and removing barriers to patient flow across the health
system.
The Patient Flow Nurse Coordinators role provides clinical oversight for complex transfer cases,
including high-acuity and behavioral health patients, and serves as a key resource for real-time
decision-making related to patient placement and throughput. The Patient Flow Nurse Coordinator also
functions in an expeditor capacity, proactively identifying and resolving barriers to discharge and
patient progression.
In addition, this role supports transfer center operations and bed coordination during periods of high
volume, ensuring safe, efficient, and timely patient movement across the continuum of care.
QUALIFICATIONS:
Education & Experience:
• Bachelor of Science in Nursing (BSN) preferred.
• Minimum of 5 years of clinical nursing experience, preferably in behavioral health, critical
care, or ED
• Experience working in a command center or centralized hospital operations setting
(preferred).
• Experience in patient flow, bed management, or hospital operations (preferred)
• Experience in behavioral health or high-acuity patient management (preferred)
Certification/ Licensure:
• Current Registered Nurse (RN) license required.
• Bachelor of Science in Nursing (BSN) preferred.
• BLS required.
• CEN, CPEN, CCRN, or PMHN preferred

Position Responsibilities
High-Acuity & Complex Transfer Coordination
• Provide clinical oversight and coordination for high-acuity transfers, ensuring appropriate
level of care, timely placement, and safe patient movement.
• Support coordination of behavioral health transfers, including screening, placement,
communication with receiving facilities, and addressing barriers to acceptance.
• Collaborate with physicians, nursing leadership, and transfer center staff to prioritize
transfers based on clinical urgency and system capacity.
• Serve as escalation point for complex or clinically sensitive transfer cases.
Patient Flow Expeditor
• Act as an expeditor to proactively identify and remove barriers to patient progression and
discharge.
• Monitor patient flow dashboards and worklists to identify delays related to:
• Pending discharges
• Bed turnover delays
• Clinical or non-clinical barriers (e.g., consults, testing, transportation,
placement)
• Partner with nursing units, case management, EVS, and ancillary services to facilitate
timely movement of patients.
• Support achievement of key throughput metrics (LOS, discharge timing, ED holds) to
optimize throughput.
Capacity Management & Bed Coordination Support
• Collaborate with Bed Coordinators and Patient Flow Supervisors to optimize real-time bed
utilization across the system.
• Provide clinical input into patient placement decisions, ensuring appropriate level of care
and safe assignments.
• Assist with bed coordination activities during high census or surge conditions.
Transfer Center Support
• Support Transfer Coordinators during periods of high call volume by:
• Assisting with intake and triage of transfer requests
• Ensure accurate and timely communication between sending and receiving providers and
facilities.
• Maintain adherence to established transfer protocols and workflows.
Communication & Collaboration
• Escalate capacity constraints, transfer delays, and patient flow barriers per OIC escalation
pathways.
• Maintain active communication with internal and external facilities to facilitate smooth
interfacility transfers
Compliance and Quality Assurance
• Ensure all transfers ahere to hospital policies, clinical protocols, and regulatory
requirements.
• Participate in process improvement initiatives aimed at enhancing transfer coordination
and improving patient outcomes
