Read MoreInspira Living Independently for Elders (LIFE) program is part of the federally recognized Program of All-inclusive Care for the Elderly (PACE). Inspira LIFE provides health that seniors living at home may have access to health care, social support and more. In addition, the program ensures seniors can live independently in their homes for as long as possible.
Inspira LIFE offers seniors a personalized individual health care plan tailored to meet their ongoing health needs, including both home-based and center-based care. The program's interdisciplinary team coordinates and delivers care even during hospitalizations or when a higher level of care is needed. Inspira LIFE’s primary mission is to keep elders independent and living in their community with a good quality of life. The program supports seniors by providing resources to ease the burden on family members and incurs no out-of-pocket expenses for qualified participants. Inspira has two LIFE program centers conveniently located in South Jersey – Vineland and Williamstown.
Inspira LIFE provides comprehensive primary medical care both at the LIFE center and at home. The program focuses on preventive measures and proactive health management to ensure participants maintain optimal health. The program also offers after-hours access to care team members ensuring continuous support and attention to participants' health needs.
For those requiring specialty care or transitions to higher levels of care, Inspira LIFE coordinates these services seamlessly. Participants receive coordinated specialty care services, including cardiology, pulmonology, wound care, podiatry, dental, vision, hearing, physical and occupational therapy, and nutrition counseling.
The LIFE Program can provide social services to help identify and coordinate community resources, advocacy and case management needs, and comprehensive behavioral wellness counseling led by licensed therapists. This includes medication management overseen by a geriatric psychiatrist, ensuring emotional support and mental health care.
Scheduling and transportation are provided for appointments, ensuring that participants can easily access the healthcare services they need without any inconvenience or delay.
LIFE offers engaging daytime activities for seniors that promote socialization and mental and physical engagement. The day center provides nutritious meals, in-center support, and transportation to and from the center.
Inspira LIFE coordinates outpatient services such as labs, X-rays, and surgery. LIFE provides prescriptions, over-the-counter medications, medical equipment, and supplies with guidance from the LIFE care team.
The interdisciplinary team provides in-home visits, including nursing care and certified home health aide services for assistance with personal care.

We may also schedule a visit to the LIFE center to meet with the LIFE health team. Here, you will meet with Inspira LIFE professionals to develop your customized care plan.
To be eligible for the LIFE Program at Inspira:
You must be 55 years or older.
Live in Cumberland, Salem, or Gloucester County.
You must be able to live safely in your home with the services provided by LIFE.
You must also meet the state requirements for nursing home level of care (NHLOC), needing assistance with activities of daily living such as bathing, dressing, feeding or toileting, or with instrumental activities of daily living such as light housekeeping, transportation, and medication management.

If you are currently covered by a major insurance company, you may apply with Inspira LIFE at any time. When approved, Inspira LIFE becomes your new health insurance and provider, starting at the beginning of the month. For more details, contact an enrollment coordinator.
Nursing home level of care (NHLOC) is a standard used to determine if a patient requires the type of long-term care typically provided in a nursing home. To meet this requirement, you need assistance with at least three activities of daily living (ADLs), such as getting in and out of bed or a chair, dressing, bathing, or showering, eating, walking, or climbing stairs. Cognitive impairments, such as those affecting memory and decision-making, are also considered when determining if someone meets NHLOC criteria.
When you enroll, Inspira LIFE becomes your primary insurance provider, covering all prescriptions. No additional Medicare or prescription plans are needed.
Other than emergency care, all services must be authorized by the care team. A provider within the LIFE network must deliver these services. Participants may be personally liable for the cost of unauthorized or out of network services.
